CASE FACTS: Dr. Kesha Joshi worked for St. Luke's Episcopal-Presbyterian Hospital as an anesthesiologist from 1989 until 1996. During this entire time, Dr. Mohammed Bashiti served as Chief of the Anesthesiology (Department). The Department unitized a monthly peer review process in which all physicians on the medical staff and Certified Registered Nurse Anesthetists (CRNAs) discussed problems with patient care. The Department used Clinical Quality Assurance Peer Review forms that included problem indicators.
