The Medicaid program was established in 1965 (Mitchell 1991), and amendments to the Medicaid program instituted in 1968 required all states to include dental care for individuals under 21 years of age as part of the Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) service (U.S. Department of Health and Human Services 2000).
