INTRODUCTION
Hospital admissions among patients with CHF are a major contributor to health care costs. For this reason, ArztPartner almeda (1) developed a CHF disease management program. The program's aims were to make up deficits in health care delivery, improve quality of life, and reduce costs for the health insurance companies (e.g., by reducing the hospitalization rate [Stewart, Marley, and Horowitz, 1999; Whellan et al., 2001; Cline et al., 1998]).
