Mr. M, age 78, had polymyalgia rheumatica (PMR) on the basis of an elevated erythrocyte sedimentation rate (ESR), anemia, and bilateral shoulder-girdle aches and stiffness. His symptoms were controlled on prednisone 15 mg/day. Recently, he developed shortness of breath on exertion, nonproductive cough, and high-grade fever. The aches and shoulder stiffness returned. He became so weak that it was hard for him to perform everyday tasks. When I saw him, it was clear that he needed further workup in the hospital.
PHYSICAL EXAMINATION
