The patient, a 58-year-old man, had a 10-year history of multiple pink papulonodules on his anterior legs and the dorsal surfaces of both feet. Examination revealed clusters of moderately firm papules and nodules; some had coalesced to form plaques. The lesions had a distinctive, shiny--almost waxy--appearance. There was no scale. No pain or tenderness was elicited on palpation; however, the patient noted mild intermittent pruritus. Significant lower-extremity pitting edema was also seen. Morbidly obese, the man's medical history was significant for diabetes, hypertension, hyperlipidemia, and congestive heart failure.
