In November 1991, a 15-month-old girl in New York accidentally lodged her head in the footboard of a toddler bed and strangled.
Although thousands of children are killed or injured each year in unfortunate accidents, something made this case special.
For months before the death, the bed manufacturer failed to tell the government about dozens of warnings from parents, some of whose children also were trapped in the tubular steel headboards and footboards, the federal Justice Department has alleged.
The company finally filed a report with the Consumer Product Safety Commission in December 1991.
