Abstract

Medication errors have been a national health care item since the Institute of Medicine's (IOM) sentinel report, which states that annually 98,000 people die in hospitals due to medical errors. The Agency for Healthcare Research and Quality (AHRQ) estimates that approximately 7,000 people die from medication errors annually and that an error occurs in 19% of all doses administered, 7% of these errors have the potential to result in an adverse effect.
