Pharmacotherapy for addiction is starting to come into its own. After years of little progress but considerable controversy, new anti-addiction agents are being introduced, and more are on the horizon. Managed care will be called upon to evaluate the cost-effectiveness of these medications, and with this in mind, I would offer some observations. This is not a scientific paper, but rather reflections from long-time experience--one provider's attempt to update behavioral managed care decisionmakers on a field with which they may be relatively unfamiliar. I will address specific agents, as well as philosophies of care.
