INTRODUCTION
The recent health care reform debate has once again focused attention on the special issues surrounding the delivery of health services in rural areas. Me ability of competitive models to address health care needs in much of rural America is a matter of debate (Fuchs, 1994). Even the strongest advocates of the marketplace suggest that "managed cooperation" among rural providers may be more appropriate in many instances (Buck, 1993).
