This four-part review defines plantar fasciitis as a 'traction perostitis' of the medial band of the plantar fascia, and reviews the distribution of plantar fasciitis in various populations. Aetiology is multifactorial. A variety of causes have been cited including running, lack of ankle dorsiflexion, obesity, alteration in heel fat pad properties, calcification of the heel fat pad, systemic disease, nutritional osteomalacia, and heel spurs. The anatomical relationships causing the development of plantar fasciitis syndrome is discussed and a staging system is presented.
