INTRODUCTION
Differences in access to healthcare services and the resulting adverse health outcomes are major public health priorities. The Institute of Medicine (IOM, 2002) and the Department of Health and Human Services (USDHHS, 2000), identified the need for strategies to improve access to healthcare services and to support improvement of health outcomes (AHRQ, 2002; IOM, 2002).
