Pilot error blamed for door falling off Cullen's helicopter
AN inquiry into a mid-air incident in which a door blew off an Air Corps helicopter carrying Tourism Minister Martin Cullen has blamed pilot error.
It was established that neither the captain nor co-pilot had scanned the warning alert display, which would have indicated a cabin door was open.
The investigation team found the left sliding door, which fell to the ground during flight, had not been correctly closed and secured prior to take-off.
