You'll be surprised how many times it can be the former, even though the latter is usually the first to be blamed when humidity and discomfort problems surface in a building.
The typical insulation system consists of the insulation, attachment pins or clips, the vapor barrier (or retarder), and the adhesive or tape used to seal the joints.
Unless each aspect of the insulation system is specified and installed correctly, the system (or parts thereof) will fail.
As you might expect, a failed insulation system often leads to substantial hvac problems, including inadequate heating and cooling and significant energy loss.
