The patient was a 46-year-old white man who had a skin lesion removed from his back; the diagnosis was never obtained. He presented 20 years later with metastatic disease (exact primary unknown) to the T1-2 level of the spinal cord with a resultant paraplegia. He underwent a decompression laminectomy and radiation therapy with a further workup, which revealed no further metastases. Three days after the first course of chemotherapy, the patient presented to the emergency department complaining of abdominal back, and left shoulder pain.
