Chicago Tribune ran a series of articles on September 10, 11, and 12 on medical errors resulting in patient deaths and injuries. While the articles do address some important nursing concerns, its inflammatory headlines imply that nurses are the root cause for these terrible injuries and deaths. Unfortunately the current culture in many health care institutions, as accentuated by the Chicago Tribune series, creates an atmosphere of "blame" on the individual health care provider rather than examining the failure of the delivery system itself.
