A 54-year-old male resident of Africa was traveling in the United States when he noticed dull lower right back pain. The symptoms began 4 days prior to presentation and soon after he arrived in the United States. His back pain intensified during the next 48 hours and alternated with abdominal discomfort. He described the back pain as sharp and needlelike. He was notably restless and anxious. A soft tissue swelling of the right lumbar area was noted. He soon developed nausea, vomiting, and abdominal cramps. His temperature was 37.4 [degrees] C, and he was admitted to the hospital overnight to rule out possible bowel obstruction.
