A 40-year-old woman presented with fever, pleuritic chest pain, shortness of breath, and cough. She had a history of diffuse large B-cell lymphoma (stage IV) for which she was treated with multiple chemotherapeutic agents and peripheral stem cell transplantation (approximately 7 months before presentation). A chest x-ray examination revealed bilateral, multiple nodular infiltrates. A bronchoscopy that was subsequently performed showed clear airways. The bronchoalveolar lavage specimen obtained was submitted for bacterial, viral, and fungal cultures. Biopsy was not performed at that time because of uncontrolled coughing.
