A CONSIDERABLE AMOUNT of media attention was recently paid to an Institute of Medicine report (Kohn et al., 2000) indicating that medical errors may result in 44,000 to 98,000 deaths in the U.S. each year. A more recent study reporting on the iatrogenic effects of medical care suggested that almost a quarter of hospital deaths were at least possibly preventable (Hayward and Hofer, 2001).
