Dec. 5--TAMPA, Fla. -- Distracted and overworked health workers are most likely to cause hospital medication errors that may injure or kill patients, and hospitals are struggling to prevent the occurrences, according to a national study released Wednesday.
The study by the U.S. Pharmacopeia Center for the Advancement of Patient Safety said 368 hospitals voluntarily reported 105,600 medication errors last year that caused 14 deaths.
