A great deal of attention has been devoted to medication and other errors in hospital settings lately, prompting public outcry for improved safety measures. Many of the errors have been blamed on nurses, in spite of the fact that medication administration is a multidisciplinary job.
From a different approach pharmacy services were evaluated as part of the error situation. In a study of 1,116 hospitals that voluntarily reported medication errors, 430,586 errors were reported; this reflects 5% of hospitalized patients.
