NSW: 'Substandard' staffer miscalculated dead inmate's release date

SYDNEY, May 6 AAP - A 23-year-old Aboriginal man died in custody after his release date from a NSW prison was miscalculated by an inexperienced staffer whose work was regarded as not up to scratch, a report found today.

The inmate, whose name was not released for cultural reasons, died 18 days after he should have been released, the report by former Corrective Services Commission chairman Vern Dalton, said.

"Expressed simply, the original error ... appears to have arisen from work undertaken by an inexperienced, relatively new officer ...
