One of the persistent problems in the U.S. health care system is that doctors, nurses, pharmacists and others too often do a poor job of working together when caring for patients.

The result can be unneeded and duplicative care, errors and preventable hospital readmissions -- not to mention frustrated patients and family members.

"We've seen across the country far too many examples of the failures that result from the lack of integration and coordination," said Peter Lee, executive director for national health policy at the Pacific Business Group on Health.
