Drugs with similar names account for about 15 percent of reports
to the U.S. Pharmacopeia's Medication Errors Reporting Program. USP
has released an updated list with hundreds of confusing drug names
and more than 750 drugs that have been reported to its errors
program. The confusion is compounded by illegible handwriting, lack
of knowledge about new drugs and poor spelling, among other problems,
according to the organization. Reports of sound-alike names have
prompted some manufacturers to change the name of certain
