Article: Dana-Farber puts focus on mistakes in overdoses

Two investigations commissioned by Dana-Farber Cancer Institute have shed new light on the causes of two chemotherapy overdoses that occurred there nearly a year ago and on the staff's failure to discover the errors for 10 weeks afterward.

Dana-Farber officials yesterday released a six-page summary of the two probes, which reportedly total 65 pages. They also announced that a national quality-monitoring agency has restored the beleaguered cancer center's full accreditation following a recent reinspection.
