Among the nail-biting health care providers who treat Harvard Pilgrim Health Care's million-plus subscribers, none have greater cause for alarm than those at Harvard Vanguard Medical Associates, who provide care for nearly one Greater Boston resident in every 10.

Ninety-three percent of Harvard Vanguard's 300,000 patients are insured by Harvard Pilgrim, the giant Massachusetts health plan which state officials took over last week to stave off financial collapse.
