The 'Practical Approach' review and accompanying 'Case Series' on neurosurgery in the sitting position by Dr Domaingue1 are useful additions to the previous reports and studies on the subject. To maintain a positive central venous pressure (CVP) and reduce the risk of intraoperative hypotension, his patients are volume-loaded 'with a minimum of 10 ml per kg of colloid' and positioned 'more semi-recumbent than sitting up' with the legs positioned 'such that the feet are close to neck level'1.
There may be a further reason for maintaining a positive CVP, especially during operations where there is a high surgical risk of air embolism. Ensuring that the CVP remains positive could conceivably ...
