This is a question often asked at clinical meetings and is met by a wide variety of answers from clinical experts. Until there are data to the contrary, we shall argue that the answer should be 'No'. In short, there are inadequate clinical studies to inform us whether there is any clinically significant increased risk of endometrial cancer with the use of estrogen-alone vaginal preparations used daily or intermittently over many years. Nor are there data to warrant the addition of progestogens to vaginal estrogen regimens because of an unsubstantiated fear of endometrial cancer in association with long-term unopposed vaginal estrogens.
