We read with interest the case report1 in which Saranteas et al. described the case of an elderly patient with a chronic myositis undergoing a lateral sciatic nerve block. Because they did not clearly identify the patient's sciatic nerve with ultrasound guidance, due to the presence of similar hyperechoic structures, they made many attempts with the aid of a nerve stimulator to achieve successful localization. The authors concluded by stating that neurostimulation is superior in assuring successful nerve blocks in patients with muscle "disturbances."1
