Error rates with injectable medications surpass those for other methods of administration, accounting for about two-thirds of administration mistakes, according to the Institute for Safe Medication Practices (ISMP). The need to perform unusual steps to prepare the injectable, such as changing the drug concentration, having to enact several steps to prepare the medicine, or readying the drug outside of the pharmacy, increases the risk of error.
