Making the Transition

How FQHCs help hospitals and the communities they serve.

In spring 2007, Stamford (Conn.) Hospital transitioned several of its primary clinics-Family Medicine, Internal Medicine, OB/GYN, Pediatrics and Behavior Healthto Optimus Health Care Inc., a nonprofit, tax-exempt Federally Qualified Health Center (FQHC) in Bridgeport, Conn. The center, formerly known as Bridgeport Community Health Center Inc., is now the largest provider of primary healthcare services in southwestern Connecticut.

Additionally, in a separate merger in 2004, Monmouth Family Health Center spun off from Monmouth Medical Center in Long Branch, NJ., forming an FQHC that now serves more than 10,000 disadvantaged individuals ...
