A 67-year-old man experienced anaphylaxis after receiving protamine sulfate [therapeutic indication not stated].
The diabetic man had been treated with insulin suspension isophane containing 0.35-0.45mg protamine sulfate/100 units insulin, for 5 years. His wife denied any experience of anaphylaxis after receiving the drug. While undergoing coronary artery bypass surgery, he was weaned off bypass and administered an infusion of protamine sulfate [dosage not stated]. After two-thirds of the infusion had been given [time to reaction not clearly stated], he developed severe hypotension and bronchospasm.
The man was treated with vasopressors, corticosteroids and aminophylline. Bronchoscopy ...
