Paromomycin
Three paediatric patients developed tetany during or after treatment of visceral leishmaniasis with paromomycin.
A 4-year-old girl started receiving a 21-day course of daily IM paromomycin 0.3mL (11 mg/kg). After 13 doses, her parents reported abnormal hand postures following the injection. Examination revealed carpo-pedal spasm of both hands, flexion of her metacarpophalangeal joints, extension of interphalangeal finger joints and thumb apposition. Tetany was diagnosed. Her symptoms improved after administration of calcium gluconate. Paromomycin was continued and, at completion of the treatment course, she had a total serum calcium level of 9.6 mg/100mL She showed no evidence ...
