Paromomycin

Three paediatric patients developed tetany during or after treatment of visceral leishmaniasis with paromomycin.

A 4-year-old girl started receiving a 21-day course of daily IM paromomycin 0.3mL (11 mg/kg). After 13 doses, her parents reported abnormal hand postures following the injection. Examination revealed carpo-pedal spasm of both hands, flexion of her metacarpophalangeal joints, extension of interphalangeal finger joints and thumb apposition. Tetany was diagnosed. Her symptoms improved after administration of calcium gluconate.
