A 25-year-old man with juvenile rheumatoid arthritis and Still's disease developed autoimmune hepatitis during treatment with infliximab, which progressed to fulminant hepatic failure.
After 3 years' therapy with IV infliximab (a 300mg infusion every 3 weeks), the man presented with lethargy, poor appetite, jaundice and worsening liver function test results. On admission, his AST, ALT and ALP levels were 3575, 4521 and 121 lU/mL, respectively, and his total and direct bilirubin levels were 16.6 and 12.5 mg/dL, respectively.
