Dr. Wright: The Antihypertensive and Lipid-Lowering Treatment to Prevent Heart Attack Trial (ALLHAT) is a comparison of a dihydropyridine, a calcium channel blocker (amlodipine), an alpha-blocker (doxazosin), and an ACE inhibitor (lisinopril), each compared with a thiazide-type diuretic (chlorthalidone). In addition, there is a lipid-lowering arm in a cohort of the trial comparing pravastatin with usual care. Patients will be followed for 6 years.
ALLHAT is one of the first published trials with dual blood pressure goals, both diastolic blood pressure less than 90 mm Hg as well as systolic blood pressure less than 140 mm Hg. Other agents (atenolol, reserpine, and hydralazine) could be ...
