ABSTRACT

The development of disease management programs remains a relatively new endeavor for most health care organizations. Ensuring that the outcomes of such programs are appropriately assessed and reported is essential for determining the program's impact on patients, the overall effectiveness of the program for a given patient population, and the overall value of the program to the health care organization. This month's column highlights some of the ways to assess outcomes of a disease management program and how to report results to meet the needs of different people within the organization.
