We read the article by Turan et al1 with great interest. However, we do not understand the need for administering the entire intubating dose of rocuronium (0.6 mg/kg) before the administration of thiopentone. Rocuronium provides good intubating conditions within 60 seconds2. Had rocuronium been used in a priming dose there might have been some justification.
Turan et al reported mild pain in 10% of their patients on injection of magnesium sulphate. In a recent study we observed that pretreatment with magnesium sulphate itself caused pain in 31% of patients. Of these, 6% reported severe pain, 21% moderate, and 4% mild, respectively3.
