Cause of water gardens deaths revealed.

Insufficient operator training and maintenance likely led to the four drowning deaths at the Forth Worth Water Gardens in Texas, according to an engineering report by Houston firm Freese and Nichols Inc.

On June 16, a father, his two children and another child drowned in what's known as the Active Pool, a vessel surrounded by a high volume of falling water. One child slipped and fell in the water, and the other three died in rescue attempts.
